 General Summary

This individual will assist in the identification of targeted youth and administering of CANS
assessments; assist participants in developing and following individual graduation plans (IGP);
and coordinate/implement after-school program activities at SJC.

 Duties and Responsibilities

        Provide on-going case management services and support to assigned program
         participants, and families as needed through the following activities:
            1) Maintenance of personal and collateral contacts (CANS, IGP’s, units of credits
            2) Maintenance of case records (personal information, attendance, follow-ups, and
                report cards)
            3) Guidance, advocacy, and assistance in navigating through various systems (e.g.,
                school district, probation, social services)
            4) Regular parent contacts, and if necessary home visits
            5) Attend assigned school staff and team meetings

        Assist in the coordination and implementation of program activities including:
            1) General supervision and monitoring of homework, tutoring and other activities.
            2) Facilitation and planning of peer group sessions
            3) Delivery of youth development programming/workshops (based on individual
                 interest, skill, talent)
            4) Assistance with general activity planning and program evaluation
            5) Recruitment of program participants
            6) Registration, attendance, and daily activity sign-in

 Education Requirements

Bachelor’s degree in human services or related field preferred or Associate’s degree with 2 years
of youth development experience

 Previous Experience Requirement

At least 2 years’ experience working youth, with special emphasis on youth development

 Knowledge and Skills Needed to Perform Effectively in this Position

       Word-processing and database proficiency
       Strong leadership and communication skills
       Organizational and time management skills
       Familiarity with community resources

 Additional Requirements

       Must be fingerprinted to be submitted for criminal background checks
       Valid driver's license and clean driving record
       Reliable Transportation for daily travel between agency’s main office and program sites
       Independent auto insurance (Minimum of $100,000/$300,000 liability limits)

 Physical Dimensions (Job-related physical abilities an individual needs to possess in order to perform the job
 in a satisfactory manner)
