                  International Student Health Insurance Policy
                                  August 1, 2012


All International students in the United States on a visa who are enrolled at the
University of Tennessee, Knoxville are required to have health insurance. As a
result of this requirement, international students are automatically enrolled in the
UTK’s sponsored health plan and their Bursar’s account (VolExpress account) is
charged for this insurance at the time of enrollment.

Waivers
If you have other health insurance and wish to be waived out of the University
sponsored health insurance the following steps must be taken:

   1. You must come to the Student Health Center and provide proof of
      coverage at the beginning of each semester.
   2. The proof of coverage must be an official document from your insurance
      company. The document must be in the form of the official policy. Your
      name and the dates of coverage must be clearly stated on the document.
   3. The official document must be written in English and provide coverage
      amounts using U.S. currency.
   4. Your policy’s coverage must clearly indicate that the following
      requirements are met:

          Coverage of at least $100,000 per accident or illness
          Deductible not to exceed $500 per accident or illness
          Repatriation coverage of at least $20,000
          Medical evacuation coverage benefits of at least $50,000
          A waiting period for pre-existing conditions of less than 13 months
          Does not require the student to pay more than 25% of the covered
           benefits per accident or illness
          The insurance company underwriting the policy must have an A.M.
           Best rating of “A-“ or above, an Insurance Solvency International, Ltd.
           (ISI) rating of “A” or above, or a Standard & Poor’s Claims paying
           Ability rating of “A+” or above
   5. The official documents must be brought to the Insurance Office of the UTK
      Student Health Service before August 31st for the Fall semester or January
      31st for the Spring/Summer semester in order to be waived out of the
      mandatory insurance plan. Requests for a waiver after the stated
      deadlines will not be accepted and you will be responsible for paying the
      full semester’s health insurance cost.

Dependents
Students may purchase health insurance for their accompanying dependents via
the university. The dependent’s policy must be purchased by September 30th for
the Fall semester or January 31st for the Spring/Summer coverage. An
exception to the stated deadlines allows for “life changing events” such as
marriage, divorce, birth of a child or the late arrival of your dependents to the
United States. Official documentation of the “life changing event” must be
submitted to determine if a true exception has been met.

Graduate Assistantships
International Graduate Students who have a graduate assistantship appointment
of 25% or higher will receive the University sponsored health insurance as a
benefit paid by their employing department. The Insurance Office of the Student
Health Center will remove the student’s health insurance charges upon completion
of the Insurance Fee Waiver form and notification from the payroll department of
UTK’s Treasurer’s office.
This UTK paid benefit is for the period of employment only. At the time the
graduate assistantship is officially terminated the international student is required
to notify the Insurance Office of the UTK Student Health Center and inform them
of the assistantship termination and the need for reinstatement of remaining
health insurance coverage costs. For example if an international student receives
a 10 month assistantship from August 1 – May 31st , the student is required to
inform the Insurance office of the UTK Student Health Center of the terminating
assistantship during the last month (May) of the assistantship. The cost of the
remaining health insurance coverage will be reinstated to the International
Student’s Bursar account at a pro-rated amount by the Insurance Office of the
UTK Student Health Center. The student will be required to pay the remaining
coverage cost.
Review of Deadlines
   International Students seeking a health insurance waiver: Before August
     31st for Fall Semester
   International Students seeking a health insurance waiver: Before January
     31st for Spring Semester
   International Students seeking to purchase dependant coverage:
     September 30th for Fall Semester
   International Students seeking to purchase dependant coverage:
     January 31st for Spring/Summer coverage

Important Information Regarding Health Insurance Plan
All students must go to the Student Health Center for initial medical care. If the
Student Health Center’s physician refers you to a specialist a written referral will
be sent to the insurance company authorizing treatment to be provided by a
physician outside the Student Health Center. If the specialist desires to refer the
student to another physician, the student must return to Student Health and
obtain a second referral to the next physician. If the second referral is not
obtained the entire cost of the additional treatment will be the responsibility of
the student to pay.
