DUE DATE: Grant applications due by 4 p.m. on July 31, 2008 Mail or hand deliver (faxes or e-mail will not be accepted) the signed original applications to: Smart Women Grants C/O Zions Business Resource Center 310 S Main, Mezzanine North Salt Lake City, Utah 84101 Application format REQUIREMENTS: Submit the original application, complete with original signatures, and the cover sheet on the top. Faxes or e-mails will not be accepted. Any supplemental work samples must be clearly labeled with applicant’s first and last name, business name, and phone number. All samples and applications must be 8.5 x 11 in size. NOTE: This PDF document CAN be typed within and printed using Adobe Reader, although you CANNOT save the document electronically; please make a copy for your records.

Checklist of items which MUST accompany your application packet: Please submit this information in the following order. Incomplete applications will not be considered. Cover sheet Mission Statement – Clearly explain how your application relates to the following mission statement: “Zions Bank’s Smart Women Grants help support the efforts of everyday heroines who strengthen women, children and their communities.” Statement of Purpose – Clearly describe the focus, goals, and general direction of your work or business plan, including broad concepts or artistic objectives. Description of business – Include the following information 1. Market 2. Competition 3. Location 4. Management structure and personnel 5. Capital requirements and general budget Proposed use of grant funds – Submit an itemized list of how you will use funds if you are chosen as a grant recipient. Plan for measuring performance and results – What will be measured, how it will be measured, and how will results be shown? Financial Data and Need – Please detail your financial situation and how it relates to the need for a Smart Women Grant. Please include the following information: 1. Gross Annual income 2. Annual mortgage or rent information 3. Statement about financial need Supporting documents and work samples – Supporting documents must accompany applications and provide supplemental relevant details. Binders, folders or envelopes may be used to enclose or arrange labeled work samples. All items submitted as work samples must be clearly labeled with applicants first and last name, business name, and phone number. Application packets and samples will not be returned. Examples of supporting documents and work samples include: Resumes, legal documents, letters of reference, marketing materials, financial statements and projections, etc.
