Woodley Junior Netball Club 2012–13 Registration Form
England Netball have made changes to the way they collect information about players, in particular they have
tightened up their data management processes and as such are asking all plays to complete a form that the club
holds on EN’s behalf. To reduce the level of duplication, that this would have caused, we have combined the
EN form and the pervious Woodley Netball registration form into one.
There are now two parts to the 2012-13 form:
1) Information that is required to be collected on behalf of England Netball
2) Additional information collected for Club use
The EN section of the form is a copy and paste from the form provided by EN and as such you cannot type any
details into this section. Pages 2 onward contains the information that the club needs and as such you can type
information into these pages. Apologies for the inconvenience this will cause. It is important that you complete
all the sections on this new form and sign at the bottom of pages 1, 2 and 3.
Fees for this season must be paid before or at the registration evening and can be paid in two ways:
- Online payment (add in details)

- Cheque made payable to Woodley Junior Netball Club
Fees for 2012–13 are:
Annual membership to England Netball and Woodley Junior Netball Club - £30 plus
Training Fees: Annual Payment - £120 or 3 term payments of £50, £35, £35. (The club would be really
grateful if parents could pay annually)
Match Fees are not paid at registration as some squads are large and hence some players do not play
every week. So, these are paid in advance to your team manager before the start of the game.

Video, Zoom and Close Range Photography Consent
From time to time we have the opportunity to publicise Woodley Netball Club and its achievements in some
of the local newspapers or on our website. As part of this publicity it is likely that photographs will be used to
accompany the article in question.
It is also possible that video footage may be taken during training and/or matches to assist coaches in
planning sessions and assessing how to achieve progression for individual players within their squads.
Please tick the box if you DO NOT give permission for your daughter’s photograph to be used in any
publicity articles or videoing of training/matches regarding Woodley Netball Club.
Woodley Netball Club does not accept any responsibility for any injury or damage sustained to any individual
or their property.

CODE OF CONDUCT (2012-13)
Woodley Netball Club recognises the importance of working together with players, parents and coaches to
ensure that everybody is striving towards common goals. We have therefore prepared a Code of Conduct
to be signed by Players, Parents and Coaches. We would be grateful if you could read and sign below and
return this form as soon as possible. A copy will then be taken and returned to you.
Players will undertake to:
Arrive on time and be appropriately dressed for the session – suitable clothing, trainers tied properly
with good grip and support, sports socks, no jewellery, short nails, hair tied/clipped back neatly
Know and abide by the rules and spirit of the game of netball
Abide by the instructions of the coach and officials – do not argue with them
Respect others – treat the other participants, your coaches and officials with respect and
consideration at all times – treat them as you yourself would like to be treated
Do not use offensive language at any time
Learn to accept success and failure – win with modesty and lose with dignity
Be a good sport – applaud all good performance
Have fun, improve your skills and feel good
Attend weekly training sessions and inform your coach in advance if you are unable to attend.
Inform your coach if you are unwell or injured
Inform your coach if you need to leave the session at any time
Thank your coaches, officials and fellow participants for their support
Parents will undertake to:
Support and encourage their child to follow the Player’s Code of Conduct as above
Keep the Club informed of any factors which may effect their child’s ability to play
Advise the Club of any changes to contact details that may be used should an emergency arise
Woodley Netball Club will undertake to:
Provide a safe, fun environment in which to train
Provide qualified and experienced coaches to take the sessions
Treat players with respect
Listen to player/parental concerns and work together to resolve any issues raised
Encourage parental involvement with all aspects of the Club – coaching, umpiring, child welfare,
Provide opportunities for players to take part in matches/tournaments whenever possible
