EMERGENCY ASSISTANCE PROGRAM REQUIREMENTS
IN ORDER TO PROCESS A REQUEST, YOU MUST PROVIDE THE FOLLOWING DOCUMENTATION USING THE ENCLOSED
For Security Deposit Assistance - Copy of the lease agreement
Rental Assistance – Copy of lease agreement, eviction notice, Court Order and Docket Number of Eviction Notice
Mortgage Assistance – Mortgage foreclosure notice
Utility Assistance – Shut off notice from the utility company
For Security Deposit, Rental or Mortgage Assistance, you will also need to provide the landlord’s name, address, phone
number, FAX number and tax ID number or Social Security number.
Additional documentation includes, but is not limited to, driver’s license, photo ID, Social Security card for applicant, Social
Security cards for all family members, proof of income (e.g. pay stub), copies of receipts, money orders, etc. showing
payments made, as well as copy of any payment agreement that has been made.
