The mentor should meet with the participant at least once a month and contact them as necessary via email or
phone to monitor their progress. Please indicate which days and what times you will be available. Keep in mind
many RAMP participants are currently employed and some mentors may find it necessary to meet in the evening
or on the weekend.
RAMP participants are interested in pursuing variable career advancement programs. Some programs may take
more time than others. Please indicate how long you plan to be in the Philadelphia area.

The mentor is a very important resource for the client and the mentor should understand this before committing
to the program. Is there anything that may prevent you from fulfilling your responsibilities to the client (e.g. work
obligations, upcoming travel, or possible relocation)?
All mentors must complete and submit a criminal history background clearance. A request for Criminal Record
Check must be submitted to the Pennsylvania State Police. The request can be made online at
https://epatch.state.pa.us . When you open this page click on the menu bar toward the top, and follow the
instructions. After entering identifying information, scroll down and select “View Queued Record Check
Requests.” A payment of $10 must be made by credit card. The results of the check should appear right away.
When the screen titled Record Check Request Results appears click on the Control Number link (to the left of
your name). Then, click on the Certification Form link. Please print out the Certification Form and bring it with
you to the Mentor Orientation.
