University. The form must also be submitted in the event a faculty member cancels a class for any reason. Absences of
three days or fewer may be approved by the Department Head; however, copies of all forms must be submitted to the

MEMORANDUM

TO:          Department/Program Heads

FROM:        John L. Taylor, Dean

RE:          Absence from Campus


Attached is a revised copy of the Request for Absence from Campus Form. Please
ensure that all faculty are aware that they must complete this form for any absence
(excluding illness or accident) that does not involve consulting for remuneration outside
the University. Forms must be approved as early as possible prior to the dates of
absence.

Absence from duty because of illness or accident must be reported to the Department
Head, who in turn shall report it to the Dean's Office within three days (UHAP,
§2.06.05).

Faculty members are responsible for meeting and conducting classes at all regularly
scheduled times and places (UHAP, §7.01.01). Consequently, this form must also be
approved in advance if any class is to be canceled.

Requests for changes of class times or assigned rooms at the beginning of a semester
must be submitted in writing by the Department Head (on behalf of the faculty member
requesting the change) to the Associate Dean for approval.

Department Heads may approve absences of three or fewer working days; however,
copies of all forms must be submitted to the Office of the Dean, regardless of
length of absence.

Also enclosed is copy of the Consulting Approval Form, which replaces the Absence
from Campus form if the absence involves employment for remuneration outside the
University.
