A. Statement of Modification
HLT 120 is being moved to Social Services as is the entire Community Health Worker
Certificate Program. The revised catalog description is as follows: SSV 120
Introduction to Community Health Worker: “An introduction to public health with a focus
on outreach, including health promotion, disease prevention, advocacy, health education,
service referral, public health – social services integration, and development of effective
communication/advocacy skills.”
B. Rationale for Modification:
Since its inception, the Community Health Worker program has suffered from low and
declining enrollments as none of the coursework contained therein fulfills an associate
degree requirement, whether core course or elective. By relocating the Community Health
Worker program and making it a specialization option within both the Human Services AAS
and Social Work degree programs students with an interest in public health as part of a
larger social/human services career will be able to attain an associate degree in either
Human Services (AAS) or Social Work [Social Services (AA)] and move seamlessly into a
BAS or BS degree at the university.
C. Supporting Documentation (e.g., faculty, advisory groups, external accrediting
agencies, and/or external interest groups etc. Include appropriate documents):
A faculty working-group comprised of Martha Sutton, Dave Pettes, Mark von Destinon, and
Heather Redmond recently met with Vaira Harik, Director of the Cochise County Health
Department and Melissa Avant, Program Coordinator of Health Promotion and Disease
Prevention (STEPS Program), who represent the funding agency for the existing
Community Health Worker (CHW) program. They very strongly endorse relocation of the
CHW to Human Services/Social Work as they want student participants to be able to
acquire an associate degree and, eventually, a bachelor degree. Funding from the
Cochise County Health Department ($7,500) has been secured to assist with program
relocation and the establishment of sites for the proposed SSV 121 Social/Health Services
Practicum (formerly HLT 121 Community Health Advisor Practicum). Upon renewal of the
IGA between the Cochise County Board of Supervisors and Cochise College, on October
1, 2007 an additional $36,000 will be allocated to the College for full program
implementation.
D. Student Implications:
Students interested in public health will now have coursework available to them which is a
specialization within two associate degree programs (Human Services AAS and Social
Work AA) with potential for a fully articulated university transfer.

E. Budget Implications:
None. Relocating the CHW program to Human Services/Social Work is a no-cost item and
implementation of the new public health specialization within HS and SW will be overseen
by a proposed half-time, salaried coordinator (with benefits) funded through the $36,000
grant from Cochise County.
F. Existing Certificate and Degree Implications:
Relocation of HLT 120 and related CHW coursework will result in the movement of the
CHW program into the Social Work curriculum.

Target population:
Students interested in Public Health, Social Work, Social Services and Human Services
careers.
Projected student enrollment:
15 students Fall 2007with future growth dependent upon practicum placement in Spring
2008. The County Health Department and the College Allied Health Department have
committed to assisting with practicum placements.
Projected employment opportunities:
The Community Health Worker program required a practicum and both the Human
Services and the Social Work program require a practicum, which should help with job
placement. Possible sites for placement include: health department, nursing homes,
doctor’s offices, hospitals, social service agencies, law enforcement, prisons, jails,
juvenile detention, and the like.

Marketing ideas:
Reorganize the Social Services Advisory Committee, work to develop connections
throughout the county, in all areas of social services and health care, with the
assistance of the Cochise College Allied Health Department and the Cochise County
Health Department. Identifying practicum sites and subsequent placements builds
interest and aids recruitment.
