Please be sure to read all the information at each of the links on our “Prospective Graduate
Students” webpage ( The following is a
checklist for the return of your application. The deadline for completion of the online application
along with all supporting documentation is March 9th, 2012 if you wish to be considered for
University of Toronto fellowships.

The following documents need to be completed using the online application:

 The School of Graduate Studies online application (personal information).
 Three letters of reference from university professors who are familiar with your academic
  achievements and research interests. Reference letters can be submitted online OR mailed in
  signed and sealed envelopes to the CSB Graduate Office. Ideally, reference letters will be
  completed using the web-based reference form that is sent directly to the referee as soon as the
  applicant pays the application fee.
 A one page statement describing your interest in cell and systems biology, your goals, and
  reason(s) for choosing this department to do research. Include a list of names of potential
  supervisors and areas of research that you are interested in.
 A current copy of your resumé or CV.
 Scanned copies of academic transcripts form each university attended.
 One copy of papers published (if applicable).
 International applicants whose language of instruction at their previous university was not
  English must submit official English language facility test results (e.g. TOEFL). English
  language proficiency tests are mandatory for applicants from many countries even if the
  language of instruction was English. Please check with the graduate office.
 GRE scores are not mandatory. However, it is strongly recommended that applicants from
  outside Canada and the United States submit GRE scores from the general test and the subject
  test in Biology/Cellular and Molecular Biology/Organismal Biology.

If you receive an offer of admission, the following should be sent to: Graduate Office,
Department of Cell & Systems Biology, University of Toronto, 25 Harbord Street, Toronto,
Ontario, CANADA M5S 3G5:

 One official transcript of your academic record (from each university attended) sealed in an
  envelope and signed by the issuing institution. Transcripts received in an unsealed or
  unsigned/unstamped envelope are not acceptable. Current or former University of Toronto
  students do NOT need to provide a transcript from the University of Toronto.

Incomplete applications will not be processed until all items on this checklist, including the
application fee, have been received.

This checklist is for personal use only, and does not need to be submitted to the Graduate Office.
