Definitions and Explanations:
Site Location for Individual Shelters (Safe Rooms)
New Home Construction: Safe room will be built as home is constructed
Previously Disturbed Ground: Area that has been changed (i.e. structures built on it,
landscaping, mowing, etc.)
Previously Undisturbed Ground: Area that has not been changed (i.e. woods, pasture land not
In an existing residence: Hardening of existing rooms such as closets or bathrooms or safe rooms
that go in garages or carports
Name should be last name, first name, then middle initial
Only include your email address if it is checked on a consistent basis (i.e. every day or two)
Shelter Construction Type
New Construction: Safe room is built on-site
Prefabricated: Safe room built at another location and brought to your residence for installation
In-Ground: Partially or completely underground
Above-Ground: No part of the shelter will disturb ground
Safe Room Size: Dimensions of the safe room or square footage
Est. Number of Occupants: Number of recommended occupants based on the safe room plans
Cost Estimate Section
Safe Room Manufacturer: If built on-site, the contractors name would be listed here. If built off-
site, manufacturers name would be listed here. Please note the vendors may not be the
manufacturer, so please ask your vendor before submitting your worksheet.
Total Safe Room Cost: Complete cost including labor, installation, construction, etc.

Clear copy of a map showing site precisely located: The Cullman County Economic Development
Office - in partnership with the Cullman County Revenue Office - will provide this map. Please
notify us if there may be issues related to the map.
Photographs: Two clear photographs of the front of the residence and one clear photograph of
the project area (i.e. where the shelter will be). The project area photograph will need to include
which direction is north. All photographs should be attached or printed on an 8½’ x 11’ paper. It
is not required that the photographs are color, but they must be clear.
You are not eligible for possible funding through this program if you cannot truthfully answer all
of the check boxes in this section as “Yes”.

Section A: All work and reimbursements must be completed within a 12 month period after
notification of funding
Section E: Our office will provide a copy of your property deed to show proof of home
ownership. Please notify us if there may be issues related to your deed. Construction plans of
the safe room must be included with your worksheet. The plans must be stamped with an
Alabama Professional Engineer’s stamp or Registered Architect’s seal. If the company does not
want to provide those plans to you directly, they can contact our office. The company may send
us the drawings and we will include them with your worksheet.
