Instruction Sheet for Medical Physics Residency Application
Thank you for considering the Harvard Medical Physics Residency Program for your training.
The applicant should have earned, or will earn before the start date, a Ph.D. in physics, medical
physics, engineering or related science. If the applicant’s degrees are in fields other than physics
or medical physics, they must supply evidence that they satisfy the physics requirements of the
American Board of Radiology (http://www.theabr.org). Please note that the ‘personal statement
of professional goals’ must clearly explain why the applicant wishes to pursue a career in medical
physics, indicating any contact they may have had with a person in the field or a medical physics
program. The selection committee places great emphasis on this statement. All documentation
must be submitted electronically.
Applications must include:
(1) A completed Medical Physics Residency application form. Available on our website,
(2) Official transcripts. Submit complete transcripts from each undergraduate college,
university and graduate school you attended. Provide certified or notarized English
translations of documents written in a language other than English. If the candidate’s first
degree is not in physics, the candidate should indicate which courses meet the ABR
(3) Three original letters of recommendation. Ask three faculty members, including your
thesis advisor, who are thoroughly familiar with your work to send letters of
recommendation. Each letter should include the writer's opinion of your professional,
academic and personal qualifications, as well as his/her opinion of your potential for
success in the field for which you are applying.
(4) A curriculum vitae. Describe your activities with professional societies, publications
and any other relevant information about your education or experience.
(5) A personal statement of professional goals.

The application process will not be considered complete until we receive your completed
application form and all of your supporting documentation. The closing deadline for receipt
of all materials is December 1, 2012
