Skilled Nursing (includes assessment, teaching and intervention)
Wound care
IV Therapy
Medication management, disease management
Administration of injectable medications
Ostomy care (bowel, urinary, GI, trach, etc.)
Exacerbation of disease (i.e. CHF, COPD, Diabetes)
Post surgical care
Rehabilitation Therapy (includes assessment, teaching and intervention – PT/OT/ST)
Therapeutic exercise, gait training, ROM, home exercise programs
Assessment of home safety and durable medical equipment needs
Modalities including ultrasound, e.stimulation and wound care
Pain and symptom management
Orthopedic rehabilitation following surgery
Speech and swallowing disorders
Fall risk
Examples of Homebound Status
Generally speaking, a patient will be considered to be homebound if s/he has a medical condition that
restricts the ability to leave one’s place of residence. Absences from home should require considerable
and taxing effort, and be for medical reasons or religious services, or be infrequent, or of short duration
when for other reasons. Why a patient may be homebound:
Dyspneic with minimal activity
Complex wound
Weakness
Psychiatric symptoms
Poor vision
Frequent falls
Uncontrolled pain
Cognitive/memory impairment
Unsteady gait/balance/poor ambulation
Unable to negotiate stairs without assist
