                                ADOPTIVE FEE SCHEDULE*

Conditions applying to all adoption applicants: All interested single individuals and married
couples who apply for adoption through The Baby Fold must meet Illinois licensing standards for
foster family homes and participate in training.

Prospective adoptive families who wish to adopt an infant will need to complete the eighteen-
hour infant study group through The Baby Fold. During and after the study group, the
prospective adoptive family and The Baby Fold will be engaged in a mutual assessment to
determine the family’s suitability to adopt and to complete the adoptive home study.

Families who wish to adopt a child internationally will need to complete the twelve-hour
International Adoption Group and a Cultural Component through The Baby Fold. During and
after the study group, the prospective adoptive family and The Baby Fold will be engaged in a
mutual assessment to determine the family’s suitability to adopt and to complete the adoptive
home study.

The Baby Fold is considered a supervising agency for International Adoption. The agency
develops partnership agreements with licensed and/or accredited placement agencies that
prospective adoptive families wish to work with depending on which country they want to adopt
from. An adoption worker will complete the necessary training and home study needed to
prepare the family for adopting out of country. This worker will assist the family with creation of
their dossier and completion of needed Citizenship & Immigration Services (CIS) forms. Finally,
once the family and child return to the United States, the adoption worker will complete all
necessary post-placement visits required by the sending country.

General Information: All prospective adoptive families will develop a Family Development
Plan with their Baby Fold worker based on each family’s needs and the child they desire to adopt.

After successful completion of all required training and assessment, a prospective adoptive family
may be considered for an adoptive placement. Payment for services related to an adoptive
placement will be made through fees charged to prospective adoptive parents. Please see
attached appropriate fee schedule.

PLEASE NOTE THAT COMPLETION OF THE TRAINING AND ADOPTION HOME
STUDY DOES NOT GUARANTEE APPROVAL OR PLACEMENT OF A CHILD.

The Baby Fold neither solicits or accepts contributions from adoptive applicants during the period
of application or before an adoption has been finalized.

*The attached fee schedule applies to all adoptive home studies completed after 5/1/2008 and will
remain in effect until further notice.

            THE BABY FOLD INFANT ADOPTION FEE SCHEDULE

The following is a breakdown of the fee and payment expectations for the adoption home
study, adoption training, and post-placement supervision services.

Adoption Home study, & Training Process:                              $1800.00

       Training/Study Group

       Office Interviews

       Home visits with all family members present

       Home study completed, fee is paid within 30 days

Medical Fee/Placement Services:                                       $3000.00

       Fee which is set aside to cover any bills related to the
       birth of the child that are not covered by the medical
       card or the birth parent’s insurance. These bills could
       include co-pays for the birth parent under their insurance
       for any cost related to the birth or services related to the
       birth that are not covered by the medical card.

Post-Placement Supervision Services (usually 6 months
of supervision needed prior to finalization):                         $2400.00 - $5400.00*

       Monthly home visits, collateral contacts, case management,
       travel, case recording, report writing, supervision,
       consultation and court appearances.

       A family would pay only $2,400 if the $3,000 medical
       fee was not needed to pay any outstanding medical bills
       related to the birth of the child (i.e., the medical card covered
       both the birth mother and baby’s birth expenses).

       A family could pay over $2,400 if any of the $3,000 medical
       fee was used to cover medical expenses for the birth mother or baby

       This fee must be paid in full prior to the submission
       of the final adoption court report.

Travel required by adoption worker for families residing
outside of McLean County, once placed:                                $.43 cents/mile
TOTAL FEES:                                                $7200.00 - $10200.00

Refund of Fees would occur in the following situations:

   1. There is no refund of the $1800 home study/training fee as the adoptive family
      would already have received the training and a homestudy would have been
      completed. This is a completed service and the family is not charged a fee until
      their training and homestudy is done.
   2. There is no refund of the mileage reimbursed as the staff would have already
      traveled to the family’s home for services.
   3. The adoptive family would receive a refund of the medical/placement fee if they
      choose not to proceed with their adoption or the birth parent chooses to parent. If
      the birth parents choose to parent, then they become responsible for the medical
      fees. If the adoptive family chooses not to adopt the child placed, the subsequent
      adoptive family will be responsible for the medical fees.
   4. The adoptive family would receive a refund of the post-placement fee if they are
      unable to adopt the child placed in their care, upon Director of Family and
      Community Services’ approval.
       THE BABY FOLD INTERCOUNTRY ADOPTION FEE SCHEDULE

The following is a breakdown of the fee and payment expectations for the adoption home
study, training and post-placement supervision services.

home study:

Includes office interviews, additional training provided,
home visits with all family members present; Home study
written and submitted to Immigration, Illinois Intercountry
Coordinator and placement agency or agent after fee is paid.

Home study updates required during waiting period for                 $150.00
either CIS or Illinois Intercountry Adoption Cooridnator:

Includes home visits, additional training provided,
Case documentation, submittal to appropriate requestor

Post-Placement Services fee:

Includes home visits, collateral contacts, travel,     $1000.00 fee for up to 4 visits*,
report writing, supervision, consultation and          if more than one child

Due to State of Illinois requirements for International
Adoption, at least one post placement visit will occur within
30 days of your return to the United States if your placement

Fees must be paid in full within 30 days from when family
receives the bill from the business office, after each scheduled visit.

Travel required by adoption worker for families residing
outside of McLean County, once placed:                                $.43 cents/mile

Refund of Fees:

   1. There is no refund of the home study/training or update fee as the adoptive family
      would already have received the training and a home study and/or update would
      have been written. This is a completed service and the family is not charged a fee
      until their training and home study and/or update is done.
   2. There is no refund of the post placement supervision fee as the adoption worker
      would have completed the visit and written the supervision report for the primary
      provider to send to the Central Authority or Sending Country. This is a
      completed service and payment is not requested until the visit/report is done.
   3. There is no refund of the mileage reimbursed as the staff would have already
      traveled to the family’s home for services.
   4. If in the future The Baby Fold acts as a primary provider/placing agency, the
      family would be refunded fees based on the fee schedule they signed at the time
      of application. Any refund of funds advanced to cover fees or expenses if
      adoptions services are not completed would be returned to the family within 60
      days upon completion of delivery of services.
              THE BABY FOLD ADOPTION FEE SCHEDULE
        FOR ADOPTIVE PLACEMENT THROUGH ANOTHER SOURCE

The following is a breakdown of the fee and payment expectations for the adoption
homestudy, adoption training, and post-placement/supervision services.

Adoption Homestudy, & Training Process:                             $1800.00
      Training/Study Group

       Office Interviews

       Home visits with all family members present

       Homestudy completed, then fee is paid within 30 days

Post-Placement/Supervision Services:                                $75.00/hour

       Home visits, collateral contacts, case management,
       travel, case recording, report writing, supervision,
       consultation and court appearances.

       The adoption worker will provide the adoptive family
       an itemized bill regarding services provided to verify
       fee charged 30 days prior to final adoption court hearing.

       This fee must be paid in full prior to the submission
       of the final adoption court report.

       If the adoption is not consummated, then the adoption
       worker will provide the family an itemized bill for
       services already provided. This fee is due within
       30 days of the family receiving the bill.

Travel required by adoption worker for families residing
outside of McLean County, once placed:                              $.43 cents/mile

TOTAL BABY FOLD FEES:                        $1800.00 + $75.00/hour supervisory fee +
                                             mileage, if applicable

The adoptive family would be responsible to pay any fees charged by the placing
agency.

Refund of Fees would occur in the following situations:
  1. There is no refund of the $1800 home study/training fee as the adoptive family
      would already have received the training and a home study would have been
      completed. This is a completed service and the family is not charged a fee until
      their training and home study is done.
   2. There is no refund of the $75/hour supervisory fee as the service would already
      have been provided by the agency.
   3. There is no refund of the mileage reimbursed as the staff would have already
      traveled to the family’s home for services.


Your assigned adoption worker will provide you the appropriate adoptive fee
schedule when you meet. You, your worker and the adoption supervisor will all
sign/date the document. One copy will be filed in the agency file while you can keep
the other copy for your records.
