•Responsible for assisting patients with special
needs such as: physical disability, literacy
problem, language barrier, hearing or visual
impairment, etc.
•Persons referred to this area for the same
process as in the Video Areas
•Please refer to “Video Area” job description

Staff will need to have packets of
forms available in different
languages
Staff will need to be prepared to
show the video in different languages
Interpreters will need to assist non-
English speaking persons in
completing the forms.

Staff will need to be prepared to
assist low literacy persons
Staff will need to check each form for
completeness and accurate
  If a person has answered
Yes or Maybe to any of the conditions
listed on the Medical Screening and
Consent Form: Refer them to the
Medical Screening and Consultation
Area or they may choose to exit the
clinic.
No to any of the conditions listed on
the Medical Screening Form; direct
them to the Vaccination Area with
their forms
Signs may be posted directing
persons to either the Medical
    Screening Area or the
      Vaccination Area.
