Maternal Child Health Outreach Coordinator
Safe Space believes that all children deserve to grow up healthy and safe, and we work to ensure
this through early intervention. Each year, Safe Space works with over 10,000 of New York
City's most disadvantaged children and families to prevent foster care placement, support and
strengthen families and promote self-sufficiency.
The mental health needs in the communities we serve are as varied as the individuals that make
up our communities. As such, Safe Space provides various services to meet the needs of our
communities. These services include Multi-systemic Therapy (MST), a well-researched
community-based treatment model directed at youth involved in criminal activity and their

The Maternal Child Health Outreach Coordinator will perform assigned duties/tasks as related to
the contracts under the Community Health Services division and it main focus will be to act as a
liaison between family support services and youth services. Individual will be responsible for the
preparation and submission of all CHW program reports and budgets. Provides weekly
clinical supervision to staff of three Community Health Workers, and one part-time
Outreach Worker and ensures that all case records are up to date and complete with all
necessary forms and documents.
Maintain up to date supervision records, case coordination summaries, team meeting
logs and other records as required as per program guidelines
Provides health education training to staff and community
Screens new clients and makes appropriate case assignment based on criteria.
Coordinates intake between HFJ and CHWP
Develops and maintains linkages with an array of community programs; Network and
establish linkages with health and service providers, community leaders, churches,
schools and consumer groups
Identifies community needs for Maternal/Child Health issues and collaborates with
consortiums and community boards to address gaps in service
Oversee Epi-Info data management and HFJ’s MIS; analyze data relative to work plan
objectives, outcomes and program development
Implements quality assurance measures as needed to ensure quality service delivery
including occasional “shadowing” of CHWs and FAW on home visits, assessments and
during outreach activities
Work on special projects surrounding the program as assigned
Provide weekend and late night coverage to staff as needed
Other duties as assigned
Education: MSW, Masters in Public Health, Nursing, or related discipline preferred.
Knowledge/Skills: Ability to provide clinical oversight to Community Health
Workers/Family Assessment Worker. Familiarity with Jamaica, Queens area and
population. Culturally sensitive. Bilingual (English & Spanish) a plus.
Experience: Minimum of two years experience required.
