To: London HIV Providers within the London HIV Consortium


Following the recent proposal from the Drugs and Treatment Sub Group of the London
HIV Consortium to support utilization of the B*5170 halpotype test, I am writing to
inform you of the following outcomes:

        The Consortium has agreed to fund the B*5170 test from December 1st 2005.
        The test and its usage will be a clinical decision based on the current evidence
         and the needs of the individual patient.
        The test is recommended as a guide to the chances of developing Abacavir
         hypersensitivity and Clinicians proposing its use must be aware of the
         literature involved in the decision. Negative test results must not be used to
         rechallenge patients showing signs of hypersensitivity.
        The usage of, and prescribing patterns following, the test will be monitored on
         a monthly basis in 2005-06 starting from December 2005.

Payments to Trusts will be based on the number of tests undertaken and evidence of
subsequent prescribing patterns, which we will monitor through the monthly
contractual monitoring.

Please find attached a revised schedule 4, Appendix A of the HIV Contract/SLA, which
we would ask to use as of your December return, which is due on January 27th. The
monitoring data that will be required, on a monthly basis, for new and existing patients.
The return should be sent to Gill Nicol, Information Manager, London Specialised
Commissioning Group. In line with paragraph 40.1 of the HIV Contract/SLA, this letter
is notification of a proposed contract variation. Please find enclosed a form, which I
would ask you to complete, sign and return to indicate your acceptance of its terms.

This test provides an opportunity for the Consortium and Clinicians to work together in
not only providing cost effective treatment but also in developing shared knowledge
which will undoubtedly enhance the patient experience. We would therefore like to
invite Trusts to consider undertaking an outcome-based exercise based on the attached
monitoring data for future publication.
