Dear Parents:

In our efforts to provide healthy and important outlets for the
teenagers in our community, Congregation Shaaray Tefilah is
pleased to announce the start of the third season of the Lobos
Hockey League, for high school boys. Due to limited space, the
league will be open on a first-come-first-served basis for boys in
grades 9-11, submitting completed applications. An application will
be considered complete if the information is correctly filled out, the
player’s parent or guardian signs the application attached, and the
payment of the fee is received.

The League
The league will be organized through a draft into teams of 9 boys per
team. It is intended that each team will play at least 12 regular
season games, with 2 playoff games, and 1 championship game for
those teams that have qualified for and advanced through the
playoffs. The season will begin IY’H in the beginning of November
2009 (detailed dates to follow). All games will take place on Sunday
evenings starting at 5pm with the last game ending no later than
11pm at the Lawrence Middle School. The team rosters, specific
game times, and other information will be provided at a later date via
email and website (see Schedule Information below).

All players must come equipped with their team Jersey, a hockey
stick, helmet, gloves, and groin protector. ALL STICKS MUST HAVE
Although the League will provide goalie equipment, it is
recommended that players who wish to play goalie bring their own
All participants in the league are expected to conduct themselves as
B’nei Torah, before, during, and after the games.

Everyone is requested to make sure they clean up after themselves,
disposing of all garbage in trash cans placed throughout the gym.
There is no food permitted in the gym.

Please note that while we welcome fathers and brothers of
participants to watch the games, we are unable to accommodate

The fee includes the cost of a team Jersey. Be sure to check off the
appropriate size jersey that you wear. Each team will have a different
colored Jersey, which must be worn to the game.

Schedule Information
Information regarding the hockey league will be posted on our
website, under Youth Department. Any
changes to the schedule or other pertinent information will be posted
on the website and/or emailed to the players. Please be sure to
provide an email address which is checked regularly.

The fee for participation in the League, and purchase of the jersey, is
as follows:

EARLY BIRD SPECIAL (Applications due October 12, 2009)
Shaaray Member -  $130
Non-Member      - $150

Standard Fees (Application due no later than October 22, 2008)
Shaaray Member -     $160
Non-Member -         $180
All applications must be in our hands by the due date listed NOT
postmarked by the due date. Applications received after the October
22 deadline will only be honored if there is available space.

All checks should be made payable to “Congregation Shaaray Tefila”
with the words “Lobos Hockey League” written into the memo section.

Waiver of Responsibility:

   Congregation Gates of Prayer (“Congregation”) shall not be liable in any way to Applicant or
   any invitees or guests, or to any other person or entity, for any loss, damage or expense which
   they may sustain or incur in connection with, as a result of or in any way relating to
   Applicants use of the Congregation’s Property or participation in the League, whether such
   loss, damage or expense shall result from the carelessness or negligence of Congregation, its
   agents, employees, guests, invitees or licensees. (The Congregation’s Property shall include
   any and all property to which the Congregation is entitled to utilize including the property of
   the Lawrence Middle School). Applicant and/or Applicant’s legal guardian shall indemnify,
   defend and hold harmless Congregation and its members, affiliates, agents and employees
   from and against (a) any and all claims, demands, causes of action, liability, loss, damage or
   injury to person or property, arising from or in any way connected to the Applicant’s
   participation in the League or the use of the Property by the Applicant or any of its invitees or
   guests, or arising from any careless, negligent or otherwise wrongful act or omission of
   Congregation or any of its employees, agents, contractors, invitees or guests, and (b) all costs,
   expenses and liabilities, including attorneys fees, incurred in or in connection with each such
   claim, demand, cause of action, proceeding, liability, loss, damage or injury to person or
   property brought thereon.

I hereby grant permission for my son to participate in the Hockey League,
and I agree to the terms outlined above.
