Dear Financial Aid Applicant,

Recently you should have received an email from BGSU explaining the new Cancellation of Student
Class Schedules (De-registration) Policy
(http://www.bgsu.edu/downloads/finance/deregistrationpolicy.pdf). Since this new policy becomes
effective for this upcoming Fall 2012 term, all students with unpaid balances as of August 16, 2012
will have their classes dropped and will not be permitted to move into their residence halls.

To assist in meeting your required financial obligations to avoid de-registration, Student Financial
Aid (SFA) would like to remind you that in order to utilize any of the loan offers made to you, on
your 2012-2013 award notification, all items listed on your To Do List and Holds section of your
MyBGSU Student Services Center must be complete. Specifically, all loan requirements listed
below must be completed no later than July 20th to meet the upcoming July 25th billing due date.
Students who complete the loan application process late should be prepared to pay for classes with
personal funds by the payment deadline(s). Additional information regarding your University
accounts will be sent to your FalconMail or listed on your MyBGSU Student Services Center. It is a
University expectation that you are checking both of these locations on a regular basis.

For ALL Federal Loans:

   1. Submit a FAFSA to BGSU                                www.fafsa.gov

   2. Submit all Verification and Certification
      forms requested by SFA on your To Do
      list and Holds section of your Student
      Services Center.                                      my.bgsu.edu

   3. Submit the online documents listed below for each of the respective loan programs.

    *If you do not want any of the loans offered to you, go to your MyBGSU Student Services Center
– Accept/Decline Awards link for the 2012-2013 aid year and “Decline” your award offer for each
loan you do not need. You will be required to contact SFA at a later date to reinstate the loan should
you change your mind.

Federal Direct Subsidized or Unsubsidized Loans (for all students):

Federal Direct PLUS Loan (for Parents or Graduate students only):

   1. PLUS Loan Application                                www.studentloans.gov

Federal Perkins Loan (for Undergraduate students only):

   1. Entrance Counseling Session                          www.mappingyourfuture.org

   2. “Accept” the loan through your MyBGSU
      Student Services Center – Accept/Decline
      Awards link for the 2012-2013 aid year               my.bgsu.edu

   3. Complete the Perkins Master Promissory Note
      using the link provided at the bottom of the
      accept/decline page (visible only when the loan
      has been accepted). NOTE: the National ID is
      the student borrower’s social security number.       my.bgsu.edu
