       the Tribunal must be sent to the Secretary, and if there are more than two parties,
       sufficient copies of the notice and supporting documents for all other parties must be
       supplied and the Secretary must be informed of the name and address of each additional
       party. You may include in the application, or in a separate application to the Tribunal, as
       appropriate: (a) a request for disclosure of any document or additional information about
       an application or reply; (b) a request for an early hearing of the application or of any
       question arising out of the application, and the reasons for that request; (c) consent to
       the matter being dealt with on written representations only; (d) a request for permission to
