                             Families
Wraparound Milwaukee REACH Program
The Wraparound Milwaukee Reach program provides case management services for Medicaid-
eligible youth with serious emotional needs. Referrals can be made by families, schools or
existing service providers.

FISS services families with youth ages 12 – 17, with the following behavioral issues and more:
   Truancy
    Non-compliance at home and at school
    Using drugs and/or alcohol
   FISS will conduct a free assessment and will refer families to appropriate services and
   resources. This could include referrals to:
   *Community agencies
   *School
   *FISS Services Unit In Home Program
   *DHS Delinquency Court Services
   *Bureau of Milwaukee Child Welfare

The FISS program parents will assist parents seeking court intervention but only after the family
has fully participated in FISS programming and serious concerns are still present.

Provides crisis intervention and assessment for children and adolescents:
257-7621. MUTT (MPS): 257-7777.
County Mental Health services for adults: 257-7222.

Milwaukee County Disability Services (includes adult protective services):

Milwaukee County Mental Health Division, Service Access to Independent
Living (SAIL): 257-8095. SAIL is central access point for Milwaukee County residents
requiring long term community support.

ARC Milwaukee: 774-6255. www.arcmilwaukee.org
Provides programs and services for people with developmental and related disabilities. Services
include information and assistance, case management, corporate guardianship, community
training, advocacy and community trust services.

Southeast Regional Center, Children and Youth with Special Health Care
Needs (Children’s Hospital): 1-800-234-KIDS. Provides support to families and youth
(birth to age 21) who have a long term physical, developmental, behavioral or emotional illness or
condition. Agency can assist with information and referrals, health benefits assistance, transition
planning, parent-to-parent support. May assist with guardianship under certain circumstances .

Offers shelter, counseling, and additional resources to help teens resolve
problems and ensure a more stable living situation.
All services are free.

Runaway and Teen Crisis Program: Provides temporary shelter and crisis counseling
to youth ages 11 through 17.
Counseling services are also offered to youth who are not residing in the shelter but wish
to seek services. Shelter located at 732 S 21st Street.

Family Empowerment Program: A 12 week program for families and teens to help
reduce stress and conflict, and to build communication amongst family members.

Transitional Living program (TLP): A program for 16 and 17 year old transient and
homeless youth. Teens will learn skills to help prepare them for independent living.
Teens will attend school while in the program. Group home located at 2175 S. Layton
Blvd.
