The 2012 St. Paul UMC Men’s Impact Ministry’s 1 Annual Grace Golf Challenge will be held on
September 29, 2012 at the Village Golf Club, a challenging and beautiful course in Conroe,
Texas. Because there are a limited number of slots available for the tournament, instructions for
player registration are as follows:

        Complete teams of four will be assessed the full registration fee of $350.00;
        Individual players will be assessed a registration fee of $85.00;
        Registrations will be processed on a first-come, first-serve basis;
        Registrations are required to be received by September 22, 2012.

All players must be at the course and registered by 10:30 AM. The tournament (scramble
format) begins Saturday, September 29, 2012 with a shotgun start at 11:00 AM.

9:30 a.m. – 10:30 a.m.              Practice Range, Registration and Snack
11:00 a.m.                          Tournament begins (shotgun start)
4:00 – 5:30 p.m.                    Reception/Dinner/Prize Distribution

Golf Clubs: Players must bring their golf clubs to the Village Golf Club no later than 10:30
AM on Saturday, September 29, 2012. Please ensure that your clubs have your name on them
to facilitate their prompt placement on the carts by the valets. Rental clubs are available.

Forms must be received by September 22, 2012. Full registration for this tournament must
be received on or before September 27, 2012; with final payment due by September 29,
2012.

Silver                   Four member team and advertisement of sponsored hole              $ 450.00
Gold                     Two four member teams, sponsored hole advertisement and           $ 750.00
                         luncheon advertisement

Confirmation letters will be emailed to all confirmed players and those persons on the stand-by
list. Completed forms and credit card information should be emailed to:
