Community Based Housing (CBH) Preliminary Application Form (2006) The CBH program presents an opportunity to expand the range of housing options in the community for people with disabilities. The program is designed to fund integrated housing units that meet the housing needs and preferences of frail elders and other people with disabilities who are institutionalized or at risk of institutionalization, for independent housing integrated into the community. CBH is intended to expand the housing choices available to these populations. Priority will be given to projects that expand opportunities for more integrated housing for people with disabilities by providing rental or condominium units in larger housing developments that are accessible and visitable. MRC and EOHHS prefer that most, but not all of the units to be developed with CBH financing, be accessible for persons with mobility impairments. All units should be affordable to persons who are extremely low income. All developers should review the Principles for Developing Integrated Housing for Persons with Disabilities and the Achieving Access and Function in Affordable Housing in Massachusetts: Part One: Developer’s Overview as well as the Program Guidelines to insure that the proposed project achieves the program and design goals of the CBH program. Please complete the pre-application and deliver to Margaret Dionne at MA Rehabilitation Commission (MRC) (Margaret.Dionne@state.ma.us) and Sara Barcan at CEDAC (email@example.com).
