It is the intention of Next Star Communications to staff our organization with honest people of integrity. Therefore, we ask you to read and understand the statement below before signing and dating
your application.
The information I have provided in this Application for Employment is true, correct and complete. False, incomplete or misrepresented information of any kind will be sufficient cause for my
application to be rejected or, if discovered after I am employed, cause for immediate termination of my employment.
I authorize the employer to contact and obtain information about me from previous employers, educational institutions and “references” I provided, and any other party necessary to verify the
accuracy of information I disclosed in this application, a related employment resume or a personal interview. To assist in the processing of my Application, I waive all rights and claims I may

The Fair Credit Reporting Act requires us to advise you that a routine investigation consumer report may be conducted relative to your character, general reputation, personal characteristics,

understand that my continued employment is contingent upon my providing the necessary documentation within the prescribed time frame.
Business needs may at times make the following conditions mandatory: overtime, shift work, a rotating work schedule, or a work schedule other than that for which I was originally hired. I
understand and accept these conditions of continuing employment.
We endorse a DRUG – FREE ENVIRONMENT and we reserve the right to drug screen all applicants and, if hired, to randomly perform drug testing.
