This application is for one professional development activity that will occur between January 1, 2013 to
June 30, 2013. Please review the “Professional Development Grant Submission Procedures” prior to submission.

1. What professional development (PD) activity are you proposing?*
Describe the activity and include the following information: the sponsoring organization(s); intended
audience; topics or content being offered that are relevant to this proposal; the nature of your involvement in
the activity (e.g. attendee, presenter). Also provide a Web site for the event/activity and attach / include a
copy of the general program schedule or agenda.
2. Why are you interested in pursuing this PD experience?
Describe the professional development needs that would be met by this activity, how you expect to benefit
from the experience, and why this particular activity is an appropriate choice for you in your UW-Madison
work role.
3. How do you plan to use and share the information you learn from this PD experience?
