      Worker was falling trees using a FL85 falling head. While falling a fir tree, approximately 28” in
      diameter by 100’ tall, the operator lost control of the tree which fell backwards, striking the boom
      of the machine and ripping off one light.

      The tree did have an uphill sweep which the operator did not notice or discounted. Note: Side-hill
      was steep, but not exceeding limits of the machine (20%). Because of the steepness, the operator
      decided to reach for the tree which put the boom at maximum reach giving the operator no
      options as far as mobility. When the tree started to fall, he had no ability to push the tree and
      because of the steepness and maximum reach, he could not lift the tree. The operator did swing
      which avoided the tree hitting the cab or house which would have resulted in significantly more
      damage. The operator has a long history running the machine, but is new to falling head and still
      in learning curve.

      Immediate Cause:
          Improper use of equipment
          Improper loading/placement
          Awkward position
          Environment

      Basic Causes:
          Inadequate planning
          Unexpected behavior
          Lacking experience

      Recommended Preventative Action:

           Get the machine planted/positioned in a good operating posture before falling tees so that
            you can use all functions. If in doubt, leave suspect trees for hand faller.
