									    Student Teaching Portfolios & Folders are due on October 5, 2009

Portfolio and Student Teaching Folder
To apply for student teaching, you must submit your School of Education folder and your portfolio by
February 15, 2010. Physical Education majors submit your materials to Dr. Koch. Health Education and
Dual majors submit your materials to Professor Gail Weintraub. The folder and portfolio must be divided
into sections (as listed below) that are neatly labeled and separated with marked dividers.

                                           Requirements

First:
   Go to the following web site: soeport.cwpost.liu.edu (do not type http//or www in front of the address)

   On the left side of the page, click on the Health, Physical Education and Movement Science link.

   On the left side of this page, click on the Student Teaching Placement link.

   On the left side of this page, click on and then download and print out: One (1) copy of the Medical
    Evaluation Form; two (2) copies of the Personal Data Sheet; and two (2) copies of the Personal
    Evaluative Statement. The medical form and evaluations will be submitted on the date identified
    below.

Place the following in the School of Education Manila Folder (to be
handed in February 15 with your portfolio):
   Two copies of the completed Personal Data Sheet

   Completed Department of Health, Physical Education and Movement Science application, which must
    include your current address, phone number and email address.

             MAJOR GPA AND OVERALL GPA MUST BE INCLUDED ON
    NOTE: YOUR
    THIS FORM. Complete the entire form as requested.

   Placement selection form

   Value of Teaching Statement-Identify why you value teaching and why you want to be a

         teacher.

   Current resume

   Current transcript

   Philosophy of teaching Health and/or Physical Education (K-12)

   PE 200 testing results for PE and Dual H & PE students
Place in Portfolio:

     Copies of items 1-8 above

     Copies of the results from an/or verification of your 1) LAST exam; 2) ATS-W Exam; 3) Content
      Specialty Exam; 4) Oral Competency; 5) Library Competency; 6) Technology Competency; 7) Child
      Abuse Seminar; 8) Violence Prevention Seminar; 9) Fingerprinting.

     Progress in meeting Beginning Physical Education/Health Education Teacher Standards

     Unit Plan

     Samples of significant work, including a writing sample with references and a research sample




Submit to Dr. Koch by April 1, 2009

     Your completed Medical Evaluation

     Two letters of recommendation, written on the Personal Evaluative Statement Form




II.        Interview Process
When you submit your portfolio and folder, you must also see Jane to sign up for an interview time. You
cannot be recommended or placed at a student teaching site until the departmental interview process has
been successfully completed. For your interview, please dress as if you are being interviewed for a job by a
school district.



FAILURE TO SUBMIT YOUR MATERIALS ON TIME WILL RESULT IN A DELAY IN
ARRANGING FOR YOUR STUDENT TEACHING PLACEMENT
