                      CONSULTANT AGREEMENT

This is consultant agreement made this __________ day of ____________________
between the Tuba City Unified School district #15 and ____________________
hereinafter      referred        to      as      the      consultant,        to        provide
_________________________________________________________ (type of services).
   1. The Tuba City Unified School District agrees to pay the consultant a rate of
       $__________ for a total of _______________ (duration).                     The amount
       $__________ will be authorized by a purchase order. The consultant shall bill
       the school after he/she completes the agreed upon service(s) and it is approved by
       the supervising administrator. The Tuba City Unified School District agrees to
       provide    the     appropriate   and   agreed   upon   facilities   for    a   term   of
       _______________ on ________________ for the benefit of Tuba City Unified
       School District.
   2. This agreement constitutes the entire agreement between parties and there shall
       be no waiver of any rights, agreements or conditions herein. No modifications or
       additions shall be binding upon either of the parties unless in writing and signed
       by both parties. A purchase order authorizing this agreement shall be issued
       prior to the commencement of services.
