Vote below yes or no for each option. PLEASE READ THE INSTRUCTIONS
CAREFULLY. Fill out the information above and email the completed form to
George Stavros at George_Stavros@toc.org, or print it, fill it out and fax it to me
with a cover sheet at 503.603.4399 or by mail to 6046 SW 25 th Avenue, Portland,
OR 97239-2081.

This is a list of group activity options being considered for our 2009 reunion in
Jacksonville, Florida. Other local attractions you can visit on your own will be
separately listed in the BOUNCE and reunion information. We are asking all
members who plan to attend the reunion to vote on every one of these options.
PLEASE DO NOT VOTE IF YOU HAVE NO INTENTION OF COMING TO THE
REUNION. We want to gauge the kind of participation we can expect before
reserving the activity. The prices are approximate PER PERSON and do not
include bus transportation that will be provided. Discounts may be applied
depending on the number of people who participate and sales tax and other
charges may be added. These are ball park figures of what we expect the costs
to be. Food/meals not included unless indicated.

Participating in this survey DOES NOT commit you to the activity or make a
reservation. We are providing this information now to ask YOU what activities
YOU want for your reunion and which you would pay to participate in. PLEASE
VOTE FOR EVERY ACTIVITY THAT YOU WOULD LIKELY PARTICIPATE IN IF
OFFERED, NOT JUST YOUR FAVORITES. Depending on the response, some
activities may be dropped, including some you favor, so vote Yes or No on
EVERY option. Some options and attractions may be combined if practical and
there is sufficient interest. If you will likely be accompanied by a spouse or other
persons, please indicate how many people will be participating. This list is far
more than the list that will actually be offered in order to give us the largest
possible range of choices for a variety of interests. A website link is provided for
each activity if you want to get more information before voting.

Please respond as soon as possible and no later than December 31, 2008.
