□      All the poems to be included in the anthology.
□      a contents page, which lists the title and author of each poem and its source (i.e.
       title of book from which it came, author/editor, publisher and date)
□       Foreword to the anthology (maximum 500 words)
□      A completed entry form.
□      Approximate costing of permissions for poems in the anthology (Within £2000 budget*)
□      entry fee (£20 per entry, cheque to be made payable to the Poetry Book Society)
□      I have read and understood the terms and conditions.
