I understand that there are risks associated with handling animals. Therefore, I
agree to assume the sole responsibility for and agree to indemnify and hold
LUV- A- PET and its volunteers harmless from any claim (including all losses,
expenses, and legal fees) for loss or injury which may be alleged to have been
caused directly or indirectly to any person or thing while performing my duties as a
volunteer.

I acknowledge that all the information contained in this form is true and correct to
the best of my knowledge. I also, hereby state that, LUV- A- PET is NOT liable for
any destruction of property, or aggressive behavior, from a fostered animal. I
understand that LUV- A- PET evaluates to the best of their ability to place with me
with a compatible animal. I understand, also, that I must be in control of my own
animals at all times and in control of any foster animal(s).

Fosters: I also state that if the foster in my residence is not working out and must
be removed, to allow adequate time for placement (up to 1 week). I hereby state
that LUV – A - PET will provide me with the basic necessities and vet care the
animal needs. This includes: food, collar, leash (if necessary) and medical costs in
the event of an emergency. By signing this agreement, I also I am also stating that
I am allowed the specified animals in my neighborhood, HOA or rental contract.
