6. BRIEF RESUME OF THE INTENDED WORK


6.1 Need for the study:


                  DantaHarsha1 is a diseases of a teeth, which affects all the people
irrespective of age and sex. Hypersensitivity of teeth is one of the leading diseases next to
Caries (Krimidanta). Patients who are suffering from this disease are unable to ingest foods
of various types such as cold , hot , sweet , sour due to hypersensitivity6. Hypersensitivity of
teeth has been studied for several years and it is reported as a painful condition that originates
when the thickness of the enamel or cement is significantly reduced. Usually the exposed
area is subjected to several kinds of stimuli, resulting in sharp acute pain. This painful
condition makes eating and oral hygiene very difficult to those having hypersensitive teeth7
i.e. Danta Harsha.
                Various treatment have been explained to cure Danta Harsha including
Kaval, Gandusha, Dhuma, Nasysa, Shirobasti, Mukhadharan etc.The routine treatment
employed today is Gandusha. Some of the studies have been carried out with encouraging
results for treatment with Gandusha. Irimedadi Taila is described in Dantarogas;3 it is
indicated in almost all Dantarogas including pain, hypersensitivity, vitiated gum tissue, black
teeth, caries. Similarly the Kalaka Churna is indicated in all Dantarogas as explained by
Maharshi Charaka in Chikitsasthanam/ Aadhyaya 26/187-188.5
                 Here an attempt is made to see the comparative efficacy of Irimedadi Taila
 Gandusha and Kalaka Churna Dharana in the treatment of Dantharsha.




 6.2 Review of Literature:

 6.3     Objectives of study:
 A)      Conceptual study of Danta Harsha.
 B)      Evaluation of efficacy of Irimedadhi taila and Kalaka Churna in Management of
         Dantaharsha.
 C)      To compare the efficacy of both the formulations in the management of Dataharsha.
 D)      To introduce a simple, safe, non-invasive, OPD level modality in the management of
         Dantaharsha.




 7.      MATERIALS AND METHODS:


 7.1      Source of data :
     A) Clinical Source   :     The patients attending the OPD & IPD of
                                Shri J.G.C.H.S.Ayurvedic Hospital,
                                Ghataprabha will be the materials of study.
 B) Literary source        :       The details of the literature of Data Harsha and Hypersensitive
                                   teeth will be incorporated in a great detail from the Samhitas
                                   and Other Ayurvedic & Modern reference Books.
  C) Drug Source        :          The Irimedadhi taila and Kalaka Churna will be prepared in our
                                   college pharmacy.


 7.2   Methods of collection of data:
                       It is a single blind clinical study where minimum of 20 patients in
 each group will be selected. The signs and symptoms will be recorded on the Proforma
 designed for the study.


A)     Inclusive criteria:
       1. Irrespective of age, sex, religion patients will be included for the study.
       2. Patients with Hypersensitive teeth will be taken for study after screening.


 B)    Exclusive criteria:
       1. Oral malignant lesions
       2. Syphilis
       3. HIV
       4. Any other systemic diseases


 C)    Study design: The screened patient of Danta Harsha will be randomly classified in to
                      two groups. Group A and Group B.
       Group A -     Irimedadi Taila, Gandusha 20 ml Twice daily (In morning and at night)
                      will be given for 21 days.
       Group B -     Kalaka Churna 5gm, dharana with Madhu will be given for 21 days.


 FOLLOW UP-
       The patients of both the group will be followed up once in week for 3 weeks.


 Assessment criteria: Assessment will be done on objective and subjective criteria
                               before and after the treatment.
Subjective criteria:
              a) Teeth Sensitive to cold
              b) Teeth sensitive to heat
              c) Teeth sensitive to sweet and sour food articles
 Objective criteria:
              a) Caries
              b) Plaques
              c) Hyperemia of gums
              d) Swelling
              e) Mobile tooth


 7.3   Does this study required any investigation or intervention to be conducted on
       patients or other humans or animals? If so please describe briefly.
       Investigations:
       1) Routine examination of blood,
       2) X-Ray and any other relevant investigations if needed


 Intervention:
       Both the formulation used for present study will be prepared in our college pharmacy
       and the same will be used daily for 21 days duration in each sitting.


 7.4   Has ethical clearance been obtained from your institution in case
       Yes. Clearance has been obtained through the ethical committee formulated in the
       Institution.
