I/We, the parent(s)/guardian(s) of the above-named individuals certify that he/she is physically qualified to participate in the Kennewick
Grid Kids Youth Football Program and hereby give approval to his/her participation in any and all of the activities of the team during the
current season. I/We assume all risks and hazards to the conduct of the activities and transportation to and from activities. I/We do
further hereby release, absolve, indemnify, and hold harmless the above-named football association, organizers, sponsors, or any of
the supervisors (any and all of them). In case of injury to my child, I/We do hereby waive all claims against the organizers, league
officers, sponsors, or any of the supervisors appointed by them. I/We likewise release from responsibility any person transporting my
child to or from activities.
IMPORTANT – WEIGHT LIMITS
Eligible players must be aged 7-13 as of September 1, 2012. Weight limit is 145 pounds for 13 year olds and 155 pounds for 7-12 year
olds. If a child fails the first weigh-in and is eligible for a re-weigh, the child will get only one more opportunity to re-weigh during one of
the 2010 regular registration sessions. No pre-weighs will be allowed. THERE WILL BE NO EXCEPTIONS. If a 13-year old weighs-in
at 156 or more and if a 7-12 year old weighs-in at 166 or more, the child is ineligible to register. Registrations will only be accepted for
players when the weight requirements are met. Age and weight specifications may change relative to final numbers of players in each
I/We understand my/our child is not insured while participating in the Kennewick Grid Kids Youth Football Program. I/We accept full
responsibility for the cost of treatment for any injury which my/our child may incur while taking part in the program.
RESPONSIBILITY FOR EQUIPMENT ASSIGNED TO PLAYER
I/We hereby agree that I/We have received a full set of “gear” to enable my/our child to participate in the youth football program. If I/We
fail to return the complete set of “gear” at the end of the season, I/We will be charged $300.00 plus attorney fees if needed.
The Kennewick Grid Kids reserve the right to refuse registration to anyone for any reason. Additionally, a player may be
removed from a team during the season for cause.

NOTICE TO PARENT / GUARDIAN
The Kennewick Grid Kids reserve the right to refuse registration to anyone for any reason.
Additionally, a player may be removed from a team during the season for cause.
Your child is guaranteed a minimum number of plays on game day provided he/she has attended
practices for the preceding week and has not been a disciplinary problem for his/her coach.
The minimum number of guaranteed plays (kick-off and kick-return count as plays) can change from
year to year, depending upon the number of players on each team and the abilities of the coaches to manage
substitutions. If your child meets the minimum play criteria and fails to get his/her guaranteed number of plays,
please notify both his team head coach and the League President. On the next game, your child should get
the required number of plays plus the plays he/she did not get the week before. (A flagrant violation may also
result in disciplinary action against said coach.) If this does not happen, notify the League President so that
immediate action can be taken to remedy the situation once and for all. That remedy may include disciplinary
action against said coach.
Requesting a Particular Team
Normally you cannot choose which team your child will play on. However, if you want your child to play
on a specific team and have had a previous immediate household family member play for that team (e.g.,
brother, sister, father, or mother), we will place your child on that team provided you show evidence on day of
signup that an immediate family member played on that team. Evidence includes a photo, team photo, team
jersey, team trophy, etc…
I understand the head coach of my child’s team is solely responsible for providing a safe practice
environment and that he/she may not be licensed to coach or be licensed in medical emergency treatment.
The league officers do not inspect practice fields nor do they supervise the coaching activities of the head
coach. I agree to notify a league officer in writing in the event that I feel either the practice field conditions are
unsafe or his/her head coach is unsafe and to allow two weeks for the complaint to be addressed by the
league. I understand and agree that league officers cannot rectify, and therefore cannot be responsible for,
unsafe conditions of which they are not aware.
