                                            Application Information

SUBMISSION DEADLINE: May 6, 2012

SUBMISSION OF NOMINATION MATERIALS: The materials noted below should be submitted as one Word or PDF file
by May 6, 2012, to ksurf@ksu.edu.

The number of Fellowship Awards depends on the availability of funds each year. Those providing references are asked to
comment on the suitability of candidates for the award.

Materials to be submitted as one Word or PDF file by the designated deadline include:

   1. Application form.

   2. Professional Achievements and Service during Doctoral Study at K-State.
      Available at: http://www.k-state.edu/grad/ksurf/.

   3. Description of Nominee’s Dissertation Research and Statement of Research Interest.
      The following information should be included: objectives of study, significance of research, potential for
      disclosure of intellectual property and commercial application, description of the methodology as related to
      achieving the research objectives, research progress to date, publications, presentations, and anticipated future
      results and their implications. Document must be double spaced. Maximum length is 1000 words.

   4. Curriculum vita.
      Nominee’s vita should include academic history, publications, presentations, awards and honors, professional
      service, professional experience, list of courses completed at Kansas State University with grades received, and
      cumulative GPA at K-State.

   5. Award Statement. Nominee’s statement, of 250-500 words, that describes how this fellowship award will make a
      difference in support of his/her research and professional development as a scientist.

   6. Research Summary.
      One paragraph statement, not to exceed 250 words, by nominee that describes his/her research and the importance
      of it that is written in terms that can be understood by the general public. This information should be suitable for
      publications (i.e., Kansas State Collegian, The Manhattan Mercury).

   7. Two letters of recommendation.
      Letters of recommendation should be submitted by the nominee’s major professor and one additional KSU
      graduate faculty member who is familiar with nominee’s research program. Please do not submit more than two
      letters. The letters should summarize nominee’s qualifications, highlight his/her exceptional achievements,
      describe his/her potential as a scientist, and indicate why nominee deserves the fellowship award. Letters must be
      submitted as PDF either by the faculty member or as part of the application packet via email to ksurf@ksu.edu no
      later than May 6, 2012.
