Assalamu Alaikum [Husbands]

As requested, please find enclosed two forms (to be completed by you), and an information sheet. May we
request you to read this information carefully, and then complete the forms. Please note that witnesses
must be adult Muslims, preferably known to you and your Spouse.

Please send the completed forms to us with:

   1. A photocopy of your Islamic Marriage (Nikah) Certificate.
   2. A postal order, or a cheque, for £150.00 (One hundred and fifty pounds sterling) payable to: 'The
       Muslim Law Shariah Council UK'

We shall then register this divorce at our offices and send you the original divorce certificate. You can
then send this original to your Spouse with any outstanding amount of Mahar and maintenance expenses.
We advise you to keep a photocopy for your own records.
