Fraternity Law, Sec. 60.G, requires that each alumni association render to the Eminent Supreme Recorder a
report of its current officers and activities for the preceding year.

Each Alumni Association President, or other designated association officer, should complete the following
information. Please type in the available fields. When completed, submit the copy for the Fraternity Service
Center to reports@sae.net.
