                                           STUDENT INFORMATION FORM
The purpose of this form is to provide useful student information to teachers writing the recommendation letter.

Note to students: The information you write on this form will be used by your teacher to a write letter for you. You should include
information that will help your teacher know more about you and your abilities and accomplishments. Please write your responses
neatly and be specific. You are welcome to attach an additional sheet of paper if necessary. After you fill out this form, make two or
three copies (If you are going to ask two ABP teachers and the ABP Counselor for recommendation letters, you will need to make 3
copies). Bring a copy to each teacher along with a copy of your most updated Résumé. Make sure you provide your ABP teachers
and counselor with the paper or online forms that are required by the universities. These forms are available on the universities’
websites.

Note to teachers: Recommendation letters provide important support for a student’s application. Students are responsible for
providing you with the university’s specific Recommendation Letter forms. The Student Access Statement below is for your own
information. Teachers can let students know if they prefer writing confidential letters.

STUDENT ACCESS STATEMENT (To be filled out by student). Students should inform teachers about the level of confidentiality they
select for the letter of recommendation. Universities prefer to receive confidential letters (Confidential means that you will not read
the letter written by your teacher). The reason for this is that universities believe teachers can be more honest if the letter is not
accessible to students. Please mark your choice below, then sign and date this form.
