The following procedure outline is intended to assist you in submitting the
appropriate paperwork for coursework credit, coursework approval, and
tuition reimbursement. If you have any questions, please don’t hesitate to
contact the Human Resources office.
Intent to Submit Form (HPS-PD2)
Complete form and return to Sharon Williams/Business Office by no later than October 1st. This
form must be completed and submitted in order to receive course reimbursement or to make a
lane change for the following school year. Reimbursement and lane changes will not be granted
if this form is not submitted on time, so you’ll want to complete and submit this form in a timely
You are under no obligation to take the course(s) once you submit this form. This form supplies
the Business Office with the necessary information to budget your course reimbursement and
lane change for the following year.
Course Approval Form (HPS-PD3)
In order to qualify for reimbursement or credit toward lane changes at the completion of a job-
related college/university or Hopkinton inservice course, you must do the following prior to the
start date of the course:
(1) As soon as you have chosen a job-related course, complete the Course Approval form.
Please use a separate form for each course.
(2) Attach an official course description that includes the tuition cost.
(3) Make and retain a copy for yourself.
(4) Submit the form and supporting documentation to the Assistant Superintendent and obtain
approval before starting the course. (If approved, a signed copy will be returned to you.)
Note that reimbursements will not be greater than the amount approved on this form, so use care
when entering the course cost information on the form. Only previously approved course credits
will count toward lane changes.
Tuition Reimbursement for Approved Courses Form (HPS-PD4)
In order to receive reimbursement or credit toward lane changes, a complete, separate form for
each course must be received in the Assistant Superintendent’s office before October 15th.
(1) Proof of completion (official grade report or transcript, or for inservice credit, a letter or
certificate of completion). Be sure that the grade report includes your full name.
(2) Proof of payment (cancelled check, receipt, or credit card statement.) Note that
reimbursements may not exceed that amount which has been approved.
