Instructions for Completing the PQQ
1. Questions to be answered in English.

2. Answer all questions carefully and as fully as possible Failure to provide complete or
   adequate evidence, supporting documentation or details where requested may result in
   your application not being considered. If a question is believed not to be applicable to
   your organisation, this should be clearly stated and an explanation provided.

3. If there is insufficient space for your response, please continue on a separate piece of
   paper, which should be clearly marked with the name of your organisation and the
   number of the question to which it relates.

4. Answer the questions specifically for your company not for the group, if you are part of a
   group of companies.

5. In assessing the answers to the following questions, the Authority will be seeking
   evidence of the Potential Bidder’s suitability to perform the services in terms of economic
   and financial standing, technical and professional ability. Qualification criteria will be a
   combination of both financial and non-financial factors and will be in accordance with
   Regulations 23 to 26 of the Public Contract Regulations 2006.

6. The term "organisation" refers to sole proprietor, partnership, incorporated company, co-
   operative, or voluntary organisation as appropriate.

7. Ensure that you complete the declaration at the end of the questionnaire. If this is not
   completed your application may not be considered.

8. Ensure that you complete the checklist of attached documents. Failure to provide
   complete or adequate evidence, supporting documentation or details where requested
   may result in your application not being considered.

9. Where supporting documents are requested please ensure they are clearly marked with
   the question number to which they refer.

10. One hard copy and two USB stick copies of the completed PQQ must be returned. Email
    submissions will not be accepted. The PQQ responses submitted on the USB stick
    should comprise of the completed PQQ’ and the corresponding information should be
    contained within the relevant folder and must include any necessary reports, accounts
    and other supporting documents which must be clearly referenced using one of the
    following formats:

11. The last date for the receipt of completed questionnaires is 2pm 5th July 2012. The
    Council may in its absolute discretion refrain from considering any applications that: are
    received after this date, are incorrectly completed or incomplete, or fail to comply with
    these Instructions.

12. The PQQ will be evaluated in accordance with the Evaluation Methodology contained in
     the PQQ documentation.

14       Communications

         All questions and queries about the procurement procedure must be sent via email to
         smtender@brent.gov.uk


15       PQQ Clarification Process

         Clarification request from potential Bidders

         A clarification question and answer process will operate during the PQQ stage.
         Potential Bidders should submit clarification questions via e-mail to
         smtender@brent.gov.uk Clarification questions received by any other method will not
         receive a response.

         Potential Bidders are urged to review the PQQ upon receipt and submit any
         clarification questions as soon as possible. The deadline for receipt of questions on
         the PQQ is 12 noon on 25th June 2012. Any clarification questions received after
         this date and time will not be answered.

         An anonymised copy of any clarification questions and the answers will be sent to all
         Potential Bidders.


         Clarification request from the Council

         The Council reserves the right to require Potential Bidders to clarify their PQQ
         submissions in writing. Any such request will be made by e-mail to the Potential

         Bidders nominated representative. Such clarifications questions do not seek to elicit
         additional information to that initially requested in the PQQ, nor give Potential Bidders
         an opportunity to correct incomplete or otherwise non-compliant submissions.


16       Procurement Timetable
         The timetable for the procurement is currently scheduled as below. Please note that
         the Council reserves the right to vary this timetable at any time at its own discretion.

Consortia and sub-contracting
Where a consortium approach is proposed a PQQ and support documentation must be
completed for each consortium member and submitted together to the address detailed in
paragraph 13 above.

Supporting documents must be clearly named with the name of the organisation, the
number(s) of the question(s) to which they refer. The consortia must be formed before
submission of the PQQ, however legal formation need not take place until and if the Council
proposes to award the consortia a place on the framework agreement.

Where the proposed prime contractor is a special purpose vehicle or holding company,
information should be provided about the extent to which the SPV/Holding Company will call
upon the resources and expertise of its members to fulfil the requirement.

The Council recognises that arrangements in relation to consortia and sub-contracting may
be subject to future change. Service providers/suppliers should therefore respond in the light
of such arrangements as are currently envisaged. Any future change in relation to consortia
and sub-contracting must be notified to the Council which will result in an evaluation by
applying the selection criteria to the new information provided in accordance with the
evaluation methodology.

Relevant information should be provided within Section B Sub-Contracting and Consortia of
this PQQ.

Freedom of Information

In accordance with the obligations and duties placed upon public authorities by the Freedom
of Information Act 2000 (the ‘FoIA’), all information submitted to the Council may be disclosed
in response to a request made pursuant to the FoIA.

In respect of any information submitted by an organisation that it considers being
commercially sensitive the organisation should:

        clearly identify such information as commercially sensitive;
        explain the potential implications of disclosure of such information; and
        provide an estimate of the period of time during which the organisation believes that
         such information will remain commercially sensitive.

Where an organisation identifies information as commercially sensitive, the Council will
endeavour to maintain confidentiality. Organisations should note, however, that, even where
information is identified as commercially sensitive, the Council might be required to disclose
such information in accordance with the FoIA. Accordingly, the Council cannot guarantee that
any information marked ‘commercially sensitive’ will not be disclosed.

Potential Bidder contact point
Potential Bidders are asked to include a single point of contact in their organisation for their
response to the PQQ. The Authority will not be responsible for contacting the Potential Bidder
through any route other than the nominated contact. The Potential Bidder must therefore
undertake to notify any changes relating to the contact promptly.

    that will meet the nutritional and dietary requirement of the schools (covering cultural /
    religious, medical requirements). (Max 300 words for each question). 35%
