Effective November 1, 2009 Lumacorp has selected Texas True Choice, one of the state’s
largest preferred provider organization networks to be our new PPO. They proudly serve
over 2,300 employer groups representing well over a 1,000,000 members statewide.

With a history of superior service and a vast provider network, TTC has risen to the top
among preferred provider organizations in Texas. The TTC motto of “whatever it takes”
accurately reflects their commitment of service to their members/clients.

       TTC is one of the state’s largest PPO networks offering more than 380 acute care
       Hospitals and 46,000 additional providers throughout the state

specialty or facility, city or by zip code. The site allows you to choose up to 10 selections
at once during your search either by physician or by facility.

For those you who may have dependents out of state or who travel out of state, for
services rendered when out of Texas, the network we will be using is Beech Street. They
can also be accessed on the internet at www.beechstreet.com Please note they are the
network for “out of state” services only.
