Checkout Agreement--Computer Loan Policies for Title V (Ed. 1/03/06) Dependant on availability, students taking the Title V computer literacy classes, will receive a laptop computer with all necessary software and a USB 2 storage device for the time they are active participants in a Title V computer class. After successful completion of both Title V computer classes and dependant on availability, computers will become the student’s computer until completion or discontinuation (not attending / not enrolled) of their college education at CSUB. Students receiving a Title V computer and software must meet the following conditions: 1. Purchase an appropriate carrying device for the computer. This could be a back pack with a protected compartment, a bag with padded sides that will protectively hold a computer, a case specifically designed as computer carrying case. The bag must be taken to the Title V office and approved by office personnel to check out a computer. 2. The student must read and understand the CSUB and 4cNet Policies of computer use (especially the AUP) available at: http://www.csub.edu/infores/usepolicies.htx These documents define the appropriate and inappropriate uses of computers at CSUB. Of special importance currently is the violation of downloading copyright music, video, etc. from sources that do not have rights to the material. The student has the responsibility to ask Title V personnel, class instructors, lab personal, etc. if they do not understand or have a question about a specific use prior to that use. Violation of these policies can range from loosing use of computers to being expelled from CSUB. 3. Personnel in the computer lab, CSUB Library Lower level, will help in solving hardware and software problems for these users. Hardware failures are covered for a period of 3 years from purchases date through an agreement with the manufacturer. 4. Students are responsible for replacement costs of USB storage and computer in case of loss or theft. Police reports must be made and copies must be given to Title V personnel. 5. Computers must be immediately returned if the student drops out of CSUB, misses 1 quarter (not including summer), if the student graduates, or if the student transfers to another college/university.
