                           Chicago Trip
                                    April 19 -21, 2012

The LHS Band will compete at the Festivals of Music this April in Chicago Illinois. The trip will
include stops at various Chicago landmarks, as well as tickets to Blue Man Group and Medieval
Times Dinner Theater. The total cost of this trip will be $500.00 per student based on 4 students
per room, not including spending money. Payments for this trip have been broken up into 4
installments of $125.00 each, and must be made on time or prior to the date due. The payment
schedule is as follows:
                            October 7, 2011 ………… Payment #1 due.
                         November 22, 2011 … … … … Payment #2 due.
                           January 20, 2012 … … … … Payment#3 due.
                          March 9, 2012 … … … … Final Payment due.
The success of this trip will depend on the total number of students signed up for the trip.
Student account balances will be posted in the band room and there will be several fundraising
opportunities for students to offset the cost of the trip. Please note the trip cancellation
procedures.

   Trip Cancellation Policy and Fees:
   As soon as the trip destination is established, the tour company begins paying non-refundable
   deposits for buses, rooms, meals, and events. Students who cancel after the first payment
   deadline will be charged according to the following schedule:

       Cancellation after 1st Payment – 10% of cost plus cost of ticketed events
       Cancellation after 2nd Payment – 25% of cost plus cost of ticketed events
       Cancellation after 3rd Payment – 50% of cost plus cost of ticketed events
       Cancellation after 6 weeks prior departure date – 75% of cost plus cost of ticketed events
       Cancellation within 14 days of departure and “No-shows” – 100+% of cost*

   *Students who cancel on or after the final deadline may actually have to pay more than the
   total cost of the trip. This is because the final cost of an overnight trip is based on quad
   occupancy (students staying 4 to a room). The cost for fewer than 4 to a room is higher. It is
   not fair to charge the remaining students more for having triple or double occupancy.

Cancellations after the Trip Intent Form deadline must be submitted in writing on
an official Liberty Band Excuse Absence Request Form. Cancellation charges will
be determined by the “date received” on that form.

The second page of this form is to be signed and turned in by September 16th, whether or
not you are taking the trip.

       understand that all payments must be made on time and that cancellation may result in a
       partial or no refund base on the cancellation procedure above. We also understand that
       the student will have opportunities to fundraise to offset their trip payments. Students
       must remain in good standing in the band to remain eligible for this trip.
