Pathways Authorization Forms Must be Signed.
Helping Hands offers Assistance to Low Income Residents of Paulding County.
1. Current Picture I.D. (D/L) GA of all adults in the household.
Income: Everyone in your household who is at least 18 years of age or older.
If they are not currently working they must come with you.
2. Social Security Cards for Adults & Children in the Household.
Will accept tax return/statement as a legal document for social security numbers.
3. Documents that show Income and Earnings for EVERYONE in the household:
Examples: Pay Stubs (most recent 30 days) (FOR EVERYONE IN THE HOME) - Child Support,
Alimony, Foster/Relative Care Pay, Other Income (specify)
Award Letters for the following: Social Security Income, SSI, Disability, VA Benefits, Unemployment,
Worker’s Compensation, Medicaid, Food Stamps, TANF
“O” Income Statement All adults NOT WORKING must sign a “0” Income Statement.
4. Provide proof of residence. MUST be brought on every visit.
Example: a current utility bill in your name (no older than 30 days) or a lease/mortgage
Or Notarized Shelter Statement - NO EXCEPTIONS - Mail is not considered Proof of Residence
5. FOR $$$$$ FINANCIAL ASSISTANCE:
Monthly Income must be more than Monthly Household Expenses.
Must have a verifiable - Crisis/Emergency/Hardship
MUST HAVE LIVED AT YOUR CURRENT ADDRESS (6) SIX MONTHS OR LONGER.
For Housing or Utility: Lease or Original Bill in your name and $ amount due.
(Deposits & Late Fees are NOT given)
We must have the detailed bill not just the stub.
You MUST come prepared with the last 30 days statements of your Monthly Household Expenses:
Rent/mortgage, lights, gas, water, internet, telephone, cable, auto loan, auto insurance and other
expenses. ALL bills paid to maintain your monthly household budget.
For RX (Prescriptions): Original prescription or transfer information & cost.
We DO NOT give vouchers for Schedule II or Restricted Drugs (pain killers, mood enhancers, etc., or
over-the-counter medicine. You may be referred to Tallatoona for their prescription program for
