Literary and Performing Artist Web Site Database Form
Thank you for your interest in our on-line artist database. We are pleased to add you to our growing list of more than 600 artists of every discipline. In an effort to create a searchable and more user-friendly online database as well as a searchable artist library in the ACI offices, we are asking for more detailed information from you than in previous years. The more detailed information supplied by the artist, the more useful these resources will be to parties interested in your work. By completing this form and submitting the required attachments, your information will be included on our web site and in our in-house library. Both resources are free and available to the public in an effort to create the largest audience for local artists. Finally, along with these resources, ACI offers weekly emails to individual artists announcing opportunities and resources that are available to them. Information such as calls to artists, audition notices, employment opportunities, public art commissions, etc. are included in this e-mail.

GUIDELINES:
This form is for individual artists at least 18 years of age living and working in Marion, Boone, Hamilton, Hendricks, Hancock, Morgan, Johnson, or Shelby County. These resources are non-adjudicated and are not intended as an Arts Council endorsement of an artist’s qualifications or abilities.
INSTRUCTIONS: Please fill out this form, save as “Artist Database_your name”, and place on a CD along with your digital images, image list, and CV/resume. Mail the CD and any additional support materials to the Arts Council of Indianapolis, See bottom of form for address. *Note: If you are just updating text only for the artist database, you may email the completed
form to us at artistservices@indyarts.org. Please include the following with this form: (Note: All items may be placed on an appropriately labeled CD.) 1 Current résumé. 2 *Images: We will begin including audio/video files for performing and literary artists on our on-line database in 2007-2008. You will be notified when that option becomes available. Currently, we have space for three digital images per artist. Please submit up to 10 images of you or your work (e.g. film stills, head shots, performance/publicity shots, album/book covers or other images that would enhance your listing.) Images should be saved on a disc at 72dpi as jpeg files and should be of professional quality. You should also include an image list with names/descriptions and photo credits if available. 3 Any supporting material such as reviews, articles, artist statement, performance programs, playbills, etc. All materials become the property of ACI, therefore, please do not send originals.
