                                   All reports need to have the company name
                                   and address. Currently the address formatting
                                   is not good. we might want to put the entire
                                   address on the same line directly below the
                                   company name
                                   when the date range option is selected from
                                   most of the reports that contain the option to
                                   pull a date range are auto populating with
                                   3/11/2010?? they should be empty or auto
                                   populate with todays date.

                                                 cash and Medical Assistance charges and
                                                 payments because this report will be used to
                                                 reconcile against the billing and the cash
                                                 report will be used to reconcile the cash
