CAMP FINANCIAL FORM
Please read, sign and return this form with your camp application
The fee to attend camp is $600.
This fee is substantially lower than the $5,500 it costs for each girl to come to camp. Expenses include
airfare, food, lodging, activities, an extensive dialogue and art program, and many new and exciting
experiences in Santa Fe. It takes a staff of 14 women and 100 volunteers to make it happen.
Each camp application must include a financial form signed by a parent or guardian along with a $100
Payment of the $600 must be made as follows:
1) A deposit of $100 submitted with the camp application. This amount will be refunded if your
daughter is not accepted.
2) The balance of $500 no later than April 15, 2012.
PLEASE NOTE: If payments are not received by the above deadlines, you will lose your place at camp. If
you decide to cancel after April 15 you will be required to pay a $100 fee and your deposit will not be
refunded in order to cover the cost of airline ticket cancellation. If deadlines are not met or instructions
not followed, Creativity for Peace will not refund any expenses incurred by applicant.
A limited number of scholarships are available. If you would like to apply for a scholarship, please
complete the scholarship application form (available on our website, www.creativityforpeace.com and
from Nancy Clayman) and send to Nancy (email@example.com) no later than January 1, 2012.

Travel to and from camp is arranged and paid for by Creativity for Peace. We are sorry but we cannot
accommodate any changes to the itinerary.
By signing this form parent agrees to the above conditions and agrees that daughter will not carry more
than $350 in shopping money to camp.
