     INTRODUCTION:

     NAME: I am [name] from in Kitgum Town. We are here to ask if you will participate in a research study conducted by
     two American universities in partnership with AVSI.

     STUDY PURPOSE: The purpose of this study is to assess the effects of the war on youth in the community and to
     evaluate the impact of NGO programs for youth. The study will be used only to understand the effects of the war on
     youth, as part of a research project, and has no other purpose. It is hoped that the information you give will help us
     improve our services to the community and to the many youth affected by the conflict.

     NO BENEFITS: The purpose of this survey is not to offer assistance or programs, however. In return for your time we
     would like to offer you a small gift after completing the questionnaire.

     TYPES OF QUESTIONS: I will ask you some questions about your education, work, health, feelings, and experiences
     during the war. Your participation is entirely voluntary and there is no need to answer any question.

     CAN SKIP OR STOP ANYTIME: You can refuse to answer the entire survey, or you can tell us when a question makes
     you uncomfortable and we will skip that question. There is no need to answer any question that makes you
     uncomfortable. If you like, you can end the interview at any time and we will still be happy to leave you this gift.

     CONFIDENTIAL: All your answers will be kept private and confidential, and the only person that will have access to this
     information is the head researcher for this study.
