                                                                       THE POSITION

The Clinic Practise Re-design (CPR)Consultant, reporting to the senior manager of Primary Health, will support teams in selecting, designing,
implementing, and monitoring quality improvement initiatives and plans. This position will be coaching and supporting all physician practices, all
disciplines, and office staff in their work to improve the client experience and office flow and efficiency. The goals of office practice redesign are
improving access, improving the patient experience of care, and improving provider work-life balance and the experience of care. This position will also
involve coaching and facilitating further integration of interdisciplinary professions with physician clinical practices. coaching support is crucial to the
success and sustainability of the Clinical Practice Redesign initiative. The practice coach is responsible primarily for recruiting and coaching physician
practices through the Clinical Practice Redesign (CPR) content and tools to assist in implementing improvements within and between practice settings and
in improving the patient experience. The practice coach is also responsible for mentoring and supporting the in-practice lead, identifying and helping to
eliminate barriers to improvement efforts, building relationships amongst care providers across multiple settings and jurisdictions, and supporting
ongoing engagement in, sustainability and spread of CPR improvement activities and outcomes.


                                                                        THE PERSON

• Bachelor degree in (clinical service) licensed health discipline with current licensure preferred.
• Education or experience related to quality and process improvement, using data for improvement and learning, knowledge exchange and transfer,
project management, systems thinking, and change management and coaching in complex adaptive environments.
• Desire for continual learning; preference for advanced education/experience related to leadership, team facilitation, coaching, community development,
health systems, policy development, and evaluation and research of programs.
• Proficient in computer software (Word, Power Point, Excel, Access, Outlook).
• Excellent written and verbal communication skills for individual, small group, and large group interaction, internally and externally.
