                                       FARMERS’ MARKET REVIEW

The Texas Department of Agriculture (TDA) and the contracting entity (CE) are required to do reviews of farmers and
farmers’ markets in the Senior Farmers’ Market Nutrition Program (SFMNP) and the Farmers’ Market Nutrition
Program (FMNP).


SECTION 1 REVIEW PREPARATION

Identifying Information

Contracting Entity (CE) Name – Enter the name of the contracting entity.
CE ID – Enter the CE identification number assigned by TX-UNPS. If unknown, leave blank.
Name and Address of Market – Enter the name and street address, including city, state and zip code, of the farmers’
market being reviewed.
Market Manager/Owner – Enter the name of the contact person for the farmers’ market association.
Contact Phone Number – Enter the telephone number of the contact person for the farmers’ market association.
Contact Address – Enter the address, including city, state and zip code, of the contact person for the farmers’ market
association.
Contact E-mail Address – Enter the e-mail address of the contact person for the farmers’ market association.


Review Information

1.1 Program being reviewed – Check the box of the program for which the farmers and farmers’ market are being
reviewed.
1.2 Date of review – Enter the date of the review.
1.3 Name and Title of Reviewer – Enter the name and title of the reviewer.
1.4 What are the market manager’s/owner’s responsibilities regarding the Program? List the duties of the market
manager/owner for the program being reviewed (SFMNP or FMNP).
1.5 Was the information correct on the farmers’ profiles? – Review the profiles. Is the contact information correct?
Is the farmers’ association name correct? Is the profile signed by the farmer and the farmers’ market association
representative?
1.6. Was the information correct on the farmers’ market agreement with the CE? Is the contact information for
the farmers’ market association correct?
1.7 Were the required posters on display? Farmers and farmers’ markets are required to display posters for each
program.

in Section 3.2 Review Conclusion.
