                                         EXCAVATION, TRENCHING OR BORING PERMIT

       Instructions:
       1. Submit this form with the basic permit application if excavation in the city right-of-way is planned.
       2. There is no fee for this permit; however other fees may apply depending upon the nature of the proposed work.
       3. This form is not required if the excavation is to be conducted in conjunction with another permitted activity such
       as a water or sewer tap.
       4. If the excavation, trenching or boring is to be conducted in the roadway surface, i.e., shoulder, parking or travel
       lanes, then compaction tests or flowable fill will be required.
