Before entering the excavation check the sides or the shoring for any defects assess
the excavation using a ladder, which will be secured at the top with rope. Have the
vert form ready next to the excavation and a second operative will lower the kit down
with a rope. Once you have all the kit the second operative will enter the excavation
to assist with the installation. Install the vert form in the correct method, which you
have be trained to do. Make shore the 4 corners of the excavation plated off to save
on concrete. Place the concrete with a 360 excavator and use the poker to poker the
concrete it is important to poker the concrete when using vert form for a good finish.
Once this is done then float off the top, and clean off your tools the concrete will cure
over night. Before entering the excavation carry out the same checks that you did on
the first day. Strip the vert form and cleaning it as you go on removing the kit out of
the excavation using a rope, once this is done make good any rough ends. Fit the
concrete ring as normal to the method statement. We must not contaminate the
blocks with any over materials only the 3mm clean will be brushed down the gaps

CONTROL:

Experienced operatives who will be supervised during the whole of the works will
carry out the works. Only experienced- trained operatives will operate plant &
equipment. All personnel within the work area will have available & wear the
appropriate PPE. A permit to pore will be used.

A vibration log will be kept. Sleeves will be tucked in to gloves and trousers will be
tucked in to boots to stop concrete from forming. Any skin contact with concrete will
be washed off.

GENERAL:

It is our intention to promote a safe working environment and to this end, we would
insist on full co-operation from all contractors and the general public. The works will
be carried out by experienced operatives who will be supervised during the whole of
the works. All operatives will be issued with personal protection equipment. All
operatives will be expected to wear any, and all personal protection equipment
provided. Operatives failing to wear the equipment or take care of themselves or
others will be reported to the office and disciplinary action will be taken. All plant
operators to hold a valid CTA card or CPCS card. Only experienced – trained
operatives will operate the plant and equipment. Statutory examinations will be
carried out on all plant, equipment with records kept, daily and weekly. Only
authorised-trained operatives will be permitted to drive any plant vehicles and mount
abrasive wheels on company machines. No excavations to commence until a permit
to dig has been issued and all services have been located. When reversing wagons
a banksman should be in attendance at all times. All excavation will be fenced off at
all times. Ladders used only for access and egress. Only trained operatives are to
enter a confined space.
